All initial comprehensive industrial medical-legal reports filed in any proceeding shall be accompanied by a copy of the billing for these services on form prescribed by the administrative director which shall indicate the physician's specialty, if any, and show separately the charges for all of the following:
(a) A review of medical records.
(b) Compiling the patient's medical history and performing a medical examination of the patient.
(c) Report preparation, including necessary research.
(d) The total charge.
The information contained on these forms shall be utilized by the administrative director to promulgate the range of medical-legal fees as required by Section 4624.
